2023 HMS Fall Athletic Information

Important Dates:

Week of May 15 -- Family ID Registration opens for FALL athletics
All physical paperwork must be handed in person to the health office by August 24, 2023.
Sept 7th -- First day of tryouts/practice - cross country, field hockey, soccer

B/G Cross Country Summer Running Plan

Summer Sports Physical Drop Off Hours:

Parents, beginning June 26, 2023, you can bring sports physicals to the health office at HHS from 8 am- 1 pm.  If dropping off out of those hours, please pull up to the horseshoe by the Gym and look for door #37. The black lock box will be installed there. Drop your paperwork into the drop box and a Nurse will submit your paperwork. If the physical is completed before school ends in June 2023, the students can bring it to the health office at HMS during the day.
All prospective athletes must have a current sports physical on file in the health office.  Physical examinations are good for 365 days prior to the date of the first practice.  If the physical has been completed more than 90 days prior to the first practice, the 90 day update form (Health History Update Questionnaire) must be completed.  It is possible that both forms will be necessary depending on the date of the physical and the date of the first practice.

Please drop off sport physical forms at the HMS Health Office during normal school hours. New physical forms must be handed in person.

Please note:  Sports physical forms are not cleared on the same day that you drop them off at the Health Office.  Sports physical forms get reviewed & cleared by our school physician.  A parent/guardian will receive a sports clearance email notification once approved by our school physician.
